1.Confirmation of required documents and application procedures

Required documents and application procedures differ depending on the child's eligibility category. For this reason, please confirm the eligibility category for each child here before starting the application.

●Required Documents for Eligible Person

・A copy of the certificate of removal of residence for Eligible Person (showing the period of residence in Tokyo), etc.

・Document designating the heirs (a statement that the parties who will inherit the right to receive 018 Support Benefit have been discussed and decided, and the names and seals of all heirs are affixed)

・Certificate of seal impression of the person who affixed his/her seal to the document designating the heir (original)

・Proof of the entire family register of the deceased Eligible Person from birth to death (original)

Q6. 【Confirmation of duration of residence in Tokyo】
Please select the applicable period of Eligible Person's (child's) residence in Tokyo from April 1, 2023 through March 31, 2024 from the following list.
*The period of residence in Tokyo to be eligible for the payment will be confirmed based on the situation as of the first day of each month. If a child is born or moves to Tokyo or moves out of Tokyo during FY2023, the child will receive 5,000 yen x the number of months he/she resides in Tokyo.
*If you do not plan to move out on an undetermined date, please register as if you do not plan to move out.
※If you do not report the (expected) date of your move out, and in fact you have moved out, you will be required to return the benefits for the period you did not reside in Tokyo.

・Eligible Person (child) must have lived in Tokyo on or before April 1, 2023 and have no plans to move out of Tokyo between April 1, 2023 and March 31, 2024.

・Eligible Person (child) was born in Tokyo on or after April 2, 2023.

・Eligible Person (child) moved into the Tokyo on or after April 2, 2023.

・Eligible Person (child) moved out of Tokyo on or after April 2, 2023.

・Eligible Person (child) plans to move out of the metropolitan area on or after the date of the requestor's request and between April 1, 2023 and March 31, 2024.

Once you have completed the form, the eligibility category and required documents will be displayed on the screen. Please check there and submit your application from the application page.

・Documents showing the fact of studying abroad(Certificate of the name of the educational institution, etc. where the student will study and the date of commencement of the study abroad program.)

・Documents that show the status of residence in Japan (in Tokyo) prior to study abroad(Copy of the supplementary record of family register, etc.)

※Proof of residence in Tokyo within the past 3 years

●In the case of an application by the guardian of a minor Eligible Person

・A copy of Eligible Person's family register

●In the case of an application by the Eligible Person's parents, etc.

・Documents showing the family relationship between Eligible Person and his/her parents, etc.

・Documents that show the residence status of the parents or other family members

●In the case of an application by an applicant OTHER THAN the Eligible Person's parents or guardian of a minor or a person designated by the parents

・Documents proving that Eligible Person has no parents or guardian of a minor to take care of him or her and that the requestor will take care of him or her and maintain his or her livelihood.

●In the case of Eligible Person resides outside of Tokyo for reasons such as study (other than overseas study), employment, or medical treatment, The applicant lives separately from the applicant.

・Documents showing Eligible Person‘s residence in Tokyo immediately prior to separation(A copy of Eligible Person's family register)

・Documents verifying Eligible Person's address on the certificate of residence

●In case of Eligible Persons born in or moved to Tokyo on or after April 2, 2023

・A copy of the certificate of residence pertaining to Eligible Person(Proof of date of moving into Tokyo and date of birth)

●In caes of Eligible Person moves out of the metropolitan area after April 2, 2023

・A copy of the certificate of removal of residence, etc., pertaining to Eligible Person(Something that shows how long you have lived in Tokyo)

●Eligible Persons who plan to move out of Tokyo after the date of application and between April 1, 2023 and March 31, 2024 From April 1, 2023 to March 31, 2024 after the date of application

・A copy of the supplementary record of family register, etc.
